It is mandatory in the state of California for phlebotomy technicians to receive their phlebotomy certification. To apply for state certification you must have documentation of completion for all of the required course work, clinical procedures (venipunctures), and passed a state approved certification exam.
In California they encourage you to apply online through the California Department of Health – Laboratory Field Services website. No application will be processed without paying a $100 non-refundable application fee.
It is now time to apply for your California issued phlebotomy technician certification and we will help you do that in 5 very easy actions.
Action 1: Go To the California Department of Health: Laboratory Field Services and open an account with a username and password.
Action 2: File an electronic application online. You will pay a ONE HUNDRED DOLLAR non refundable application fee – (Visa or MasterCard).
Action 3: At the end of your application procedure, print out the attestation page (a page that attests to the accuracy of all the info entered) and print out a duplicate of the “suggested experience documentation form”.
Action 4: Compile all proper documents which include: official transcripts, confirmation of training and experience, and certificate of completion of training program, two passport size pictures and state authorized examination. You will be mailing these documents along with the attestation page in step 5
Action 5: Mail the Attestation page from action 3 in addition to all other forms which are required for credentials as a phlebotomist from step 4 to the LFS. LFS will not review an application if an attestation page has not been received by LFS. You should have all correspondence addressed: “Attention: Phlebotomy Program”.
